This video tutorial does use CS4. I'm not sure why your smaller bubble is embedded. Just check that there is not an extra style attached to the smaller bubble layer and let me know how you go. Just check that you do not have a style attached to your rounded rectangle that is adding a white stroke to the rectangle.
